Use this service to change your address, name or phone number. It is important that we have up to date contact details in case we need to contact you.
If you do change your address and you are out of our catchment area, please be aware you may be asked to register with your nearest surgery. We do operate an appeal process to stay on the list but you must do this in writing to the Practice Manager. Your case will be discussed with the partnership and the outcome given to you based on the decision made, after which no further appeal can be made.

We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
- evidence if your name has changed due to marriage or by deed poll
