• are registered at the surgery

Before you start

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
• photographic identification and proof of address no older than 3 months

Patients under 16 years old must still provide photographic ID and can be given access via their parent or guardians record. 16 years and above must attend the surgery in person to register.
