Please let us know as soon as possible if you change your address, telephone number or name. It is important that we have up to date contact details in case we need to contact you.
Please note: If your name has changed due to Marriage or by Deed Poll, can you please provide us with a copy of the appropriate document (requirement of Department of Health).
If you do change your address and you are out of our catchment area, please be aware you may be asked to register with your nearest practice. We do operate an appeal process to stay on the list but you must do this in writing to the Practice Manager. Your case will be discussed with the partnership and the outcome given to you based on the decision made, after which no further appeal can be made.
