Eligible Flu and COVID-19 Vaccination Request

If you are eligible and would like the flu and/or COIVD-19 vaccine, please submit this form.

We are going to run the clinics at the Royal British Legion, Mickleover on Saturdays and some during weekdays and evenings at the Practice.

Housebound patients who are registered with the Practice will receive a separate communication as to when we will be vaccinating them.

Locations

Flu vaccination clinics will be held on the Saturday dates at the Royal British Legion, 30 Poppyfields Dr, Mickleover, Derby, DE3 9GB.

Other clinics will take place in the Practice, you will be advised of the location on booking.

Evening and early morning appointments will be offered to those eligible for the vaccine.

Advice and Guidance

We will be sending out appointment invitations that will allow you to book directly into an appointment slot. If you do not get an invitation via sms, please fill out this form.

Please ensure you provide the correct contact information in the form and do not call the Practice if you have submitted this form, we will contact you back through this form as a reply offering an appointment based on your options below.

Please do not call the Practice before 11am or after 2pm due to volume of calls. Housebound patients will be contacted by our nurse Kate personally to arrange your flu vaccination. Children who qualify will also be contacted separately.
