It’s easy to sign up to our online services and even easier to use from the comfort of your home or when you are out and about via our website or through the NHS app.
To register for our online services you will need to complete a registration form which can be done by either;
1). Attending the practice and compete the registration form
2). Downloading it and complete it by hand; Online-Access-Registration-form 2019, or if you require the form in large print; Online-Access Registration-form 2019 – LARGE PRINT
3). Visiting our website and complete it online at /navigator/register-for-online-services/
In all 3 instances you will need to visit the practice in person (this is for security reasons), bringing with you two forms of identification;
1 x photographic id i.e. Passport, Driving Licence, Bus Pass,
1 x proof of address i.e. Household Bill, Council Tax Bill, Bank Statement (please note it can be no older then 3 months
We will then issue your Username and Password details via your preferred way: a print out or sent as a sms (as long as we have a mobile phone number for you on the system).
Once you have your log in details you can choose from a variety of services through ‘Systm Patient Online Services’ ;
- Order your repeat prescriptions
- Make an appointment
- Cancel an appointment
- Change your contact details
- View your medical record
